They used to have a different name & different doctor Tom Carlyle. I am trying to get my medical records from 8.5-9 years ago. I am not sure how long the records are retained and also whether they would still have them since it changed hands. Trying to find out where they might be or if they may be retained digitally somewhere? All those years ago, I was hit from behind at 70mph after me and the cars in front of me were fully stopped, plowed my car right into the truck in front of me. I was diagnosed by Dr. Carlyle with whiplash, it was very painful. I have also had scholiosis for many years prior to that, upon looking at my x-rays Dr. Carlyle had also stated that my neck was also deformed from working over a desk for so many years, it is curved forward from where it should sit further back. I also have somewhat of a hunchback as I also have a large chest. I was instructed to go to come in 3 times a week for therapy, I went for a few treatments but being a single mother newly completely on my own for the first time having been laid off just a few months earlier with a 1 year old baby at a fairly new job a good distance away I was unable to miss the work. I finally saw a doctor last year after all this time who also reccomended I go to therapy. Again I am still unable to miss the work and no places are open on the weekend. Naturally his insurance was trying to get out of paying trying to say it was my fault, they moved at a snail's pace and wouldn't even give me a rental car. I had my insurance handle the whole claim and bill his insurance for everything plus my $500 deductible. Luckily my baby wasn't in the back seat, I was on my way to pick her up after work. I was eventually paid $4,500 for my car (not enough to buy a good new one like what I had) and needing the money and with Christmas a couple months away I signed the waiver not to ever sue and took the measley $1,000 for my neck injury. Had I known that the muscles on the back of my neck really would turn to "gristle" as Dr. Carlyle warned if I didn't complete the treatments and that I would have chronic pain and limited movement every day and be debilitated all these years later for the rest of my life I never would have signed that waiver and should have run right down and got me a lawyer as well. I did not pay this clinic anything, the insurance paid all of the medical bills. I am now trying to build a paper trail and case for disability. I didn't have insurance then and I don't have insurance now. I'm still working over the desk. I never received any government help either, I make "too much money"

I am VERY pleased with the care that I receive at 501. The staff is always very courteous and go, what I would consider, above and beyond for every client. You don't have to put your entire day on hold to go to an appointment. There is no long wait. Your in, out, and on schedule. I highly recommend 501 Pain and Rehab to everyone.

Called me at 8 in the morning asking if my accident left me injured. I told them I was fine, and “Tina” insisted that I go get checked out anyway since it would be free. She said if any other health facility called, to tell them I was taken care of. Sure enough I was immediately called by another group, who warned me of 501 by telling me about their pending lawsuit and insurance fraud. I followed up by checking them out online and sure enough others had the same experience. Don’t go to them.

Too bad there's not a minus star that I can give! I was in an automobile accident 12/20/17, and as soon as details became public I was awakened by a very charming woman who said that she was calling on behalf of my insurance company to see how I was feeling. She asked, "How are you doing?" I told her that I felt terrible, and that I could barely move. She said that I needed to come in as soon as possible so that they could help me. So, I had my son drive me down there, and when I filled out the paperwork I found a page saying that they were NOT affiliated with ANY insurance company, and that they were a Chiropractic "clinic". I told the receptionist that the woman who called me had said she was calling on behalf of my insurance company. She said, "Yes, sometimes Marketing gets a little carried away!" I was in SO much pain I could have cried, and I had been lured down there under false pretenses. The LAST thing I wanted was to be manhandled by some ambulance chasing chiropractor! So, we went to the ER and I was treated there by health care professionals.
